I don't have issues on my v3.8 sites for this as the report button shows for me in default feed.

Where is it included?

I use the default feed, no addon for that. I use a custom theme that is based on default. When I look at my developer site with default neutron theme, it also has the report button.Check your user group setting for each user group.Try default theme without any edits to it.Make sure you don't have any add on that is interfering.

I'm not sure what you mean. You must enable mysqli on your sites for the script as it is required so you can't use just mysql. If your current site uses mysql, change it in the server in include/settings/server.sett.php you will see the line for mysql and change to mysqli and then clear cache from file/cache but leave index.html in there.
